 This program offers multi-speaker content (varying clinical disciplines) designed to effect change in clinician competence, performance, or patient outcomes. It is expected that participating clinicians will report greater confidence in their approach to the clinic problems presented and/or describe how they will change their behavior and apply the newly acquired strategies to their practice (competence).

1. Use the CloudCME platform in order to easily access additional CME programming
2. Understand the current state of our Indiana AMG performance as well as our national AMG performance, and have awareness of our strategy for continued improvement of quality health outcomes, patient experience, value and clinician experience.
3. Identify HC as it transition into the next few years.

5. Discuss Peds Beh. Health treatment
6. Discuss Adult Beh. Health treatment
7. Identify Heart Failure treatment strategies
8. Understand current COPD screening guidelines
9. Utilize optimization tactics to use in Athena for improved practice efficiency

This activity was planned by and for the healthcare team, and learners will receive 4.00 IPCE credits for learning and change.
