Overview
There is ongoing uncertainty about the role of aspirin in VTE prophylaxis after orthopedic surgery. While evidence shows aspirin can reduce clot risk compared to placebo and may be noninferior to anticoagulants in certain lower-risk settings, other trials suggest anticoagulants are more effective, particularly in higher-risk arthroplasty patients. Variability in study design, patient populations, dosing, and duration of therapy makes it difficult to draw firm conclusions, and current guidelines reflect this by offering only conditional, low-certainty recommendations. Overall, the main gap is determining which patients are best suited for aspirin versus stronger anticoagulation, underscoring the need for more consistent, high-quality research to guide practice.

Objectives
At the conclusion of this activity, learners will be able to:

  1. Discuss the pathophysiology and risk of VTE in orthopedic surgery
  2. Review major clinical practice guidelines for VTE prophylaxis
  3. Identify best practices for VTE prevention in patients undergoing orthopedic fracture and/or repair

Accreditation
